Step 1 — Open Encounter History
From the left-hand menu, click Encounter History.
You’ll see two tabs at the top of the page:
Unreviewed — Notes that are still editable
Reviewed — Notes that have been finalized
To edit a note, stay in the Unreviewed section and select the encounter you’d like to open.
Go to Encounter History and click on the note under Unreviewed that you want to edit.
Step 2 — Edit Your Note
Once you open a note:
Make Manual Edits directly in any section of the clinical note.
To make Automatic Edits, type instructions into the text box at the bottom (for example: “shorten this note,” or “summarize the assessment”). AutoScribe will update the entire note automatically.
You can copy the note before finalizing by pressing Copy to Clipboard at the bottom left corner of the note.
Edit text manually within the note or type automatic suggestions below.
Step 3 — Finalize the Note
When you’re satisfied with your edits:
Click Copy & Mark Reviewed at the bottom of the screen.
The note will move to the Reviewed section and become locked for editing.
Press Copy & Mark Reviewed to finalize the note.
Step 4 — View Reviewed Notes
Finalized notes appear under the Reviewed tab.
These notes are read-only and represent your completed, verified documentation.
Reviewed notes are locked and ready for reference or EMR copy.
Quick Tips
Use the search bar to find specific patient names or encounter dates.
Only Unreviewed notes can be edited or deleted.
Reviewed notes are stored permanently for reference.
Always click Copy & Mark Reviewed once you’re finished editing — this ensures your note is saved securely and marked complete.
Result
All your encounters are neatly organized in Encounter History — Unreviewed for ongoing edits, and Reviewed for finalized notes ready to copy into your EMR.
How to Navigate Encounter History (Old UI)
In the old AutoScribe interface, Encounter History is where you review, edit, and finalize notes after an encounter has ended. Notes are organized by status so you can clearly see which ones still require review.
Step 1 — Open Encounter History
From the main page, select Encounters.
You will see two tabs at the top of the page:
Active
Submitted
Active encounters are notes that have been completed but are still editable. Submitted encounters are finalized notes.
To edit a note, remain in the Active tab and select the encounter you want to open.
Step 2 — Edit an Active Encounter
After opening an encounter from the Active list, you can:
Edit the clinical note directly in any section
Change the note template if needed
Generate or review the Patient Handout
Edits made here remain available until the encounter is submitted.
Step 3 — Copy the Note Without Finalizing (Optional)
If you want to paste the note into your EMR but continue editing it later, you can use the Copy option instead of submitting.
This copies the clinical note to your clipboard while keeping the encounter editable in the Active tab.
This option is useful if you need to revisit or regenerate the note later.
Step 4 — Submit and Finalize the Note
When you are finished editing:
Click Submit and Copy.
The note is copied to your clipboard and moved from the Active tab to the Submitted tab.
Once submitted, the note becomes read only and can no longer be edited.
Step 5 — View Submitted Encounters
Submitted encounters are stored under the Submitted tab.
From here, you can:
Copy the clinical note again if needed
Print or email the Patient Handout if one was generated
Submitted encounters represent finalized documentation.
Quick Tips
Only Active encounters can be edited.
Submitted encounters are locked and cannot be changed.
Use Copy instead of Submit if you plan to make additional edits later.
Patient Handouts remain accessible after submission if they were generated.
Result
Your encounters are organized by status, allowing you to clearly separate notes that still need review from those that are finalized and ready for reference or EMR use.
