How to Navigate Encounter History

The Encounter History page is where all your past AutoScribe encounters are stored. Here, you can review, edit, and finalize any note created through AutoScribe or Direct Dictation.

Step 1 — Open Encounter History

From the left-hand menu, click Encounter History.
You’ll see two tabs at the top of the page:

  • Unreviewed — Notes that are still editable

  • Reviewed — Notes that have been finalized

To edit a note, stay in the Unreviewed section and select the encounter you’d like to open.

Go to Encounter History and click on the note under Unreviewed that you want to edit.

Step 2 — Edit Your Note

Once you open a note:

  • Make Manual Edits directly in any section of the clinical note.

  • To make Automatic Edits, type instructions into the text box at the bottom (for example: “shorten this note,” or “summarize the assessment”). AutoScribe will update the entire note automatically.

  • You can copy the note before finalizing by pressing Copy to Clipboard at the bottom left corner of the note.

Edit text manually within the note or type automatic suggestions below.

Step 3 — Finalize the Note

When you’re satisfied with your edits:

  • Click Copy & Mark Reviewed at the bottom of the screen.

  • The note will move to the Reviewed section and become locked for editing.

Press Copy & Mark Reviewed to finalize the note.

Step 4 — View Reviewed Notes

Finalized notes appear under the Reviewed tab.


These notes are read-only and represent your completed, verified documentation.

Reviewed notes are locked and ready for reference or EMR copy.

Quick Tips

  • Use the search bar to find specific patient names or encounter dates.

  • Only Unreviewed notes can be edited or deleted.

  • Reviewed notes are stored permanently for reference.

  • Always click Copy & Mark Reviewed once you’re finished editing — this ensures your note is saved securely and marked complete.

Result

All your encounters are neatly organized in Encounter History — Unreviewed for ongoing edits, and Reviewed for finalized notes ready to copy into your EMR.
